Record Requests FAQs

The information that may be given out via the telephone is limited by statute. It is limited to lien existence, title type, odometer reading, tag expiration date and price estimates. Nothing else is available via the telephone

Print the OTC Vehicle Information Request form. This form must be completed in its entirety, signed by the requestor and notarized. Once completed simply email, fax, mail or bring by in person. The cost for each record is $1.00 plus any fees for certifying, faxing, mailing, or emailing etc.

Print the DPS Driving Record Request and Consent to Release form. This form must be completed in its entirety and signed by the requestor. Once completed simply email, fax, mail or bring by in person. The cost for each record is $25.00 plus any fees for certifying, faxing, mailing, or emailing etc.
